Traditionally, home health agencies haven't offered DSMES/DSMT in the home setting. However, with the rise in patients that have complex diabetes and need home health services, it's crucial to identify and escalate these patients for personalized DSMES/DSMT. This presentation aims to validate the benefits of providing DSMES/DSMT to homebound patients with complex diabetes.
