D19 - Shaping the Future of Equitable Diabetes Care: The Diabetes and School Health Program

Type 1 diabetes affects all ages/ethnicities but is commonly diagnosed in children. Because they spend the majority of their time at school, a large portion of the T1D care provided for children is the responsibility of school staff. The Diabetes and School Health (DASH) program is a school-based equity initiative which aims to optimize T1D care in schools and reduce healthcare disparities. As T1D school-based programs are limited, this session will introduce a unique program within a large organization that aims to enhance care coordination, improve self-efficacy, and provide education to schools and families. Presenters will share outcome/quality improvement measures, patient-centered stories, and lessons learned.
